Synopsis

Diabetic foot ulcers are among the most serious complications of diabetes mellitus, significantly increasing both morbidity and mortality. Approximately 15–25% of diabetic patients experience a foot ulcer at least once in their lifetime. When not properly managed, these ulcers may lead to severe complications such as infection, sepsis, gangrene, and major amputations. A multidisciplinary approach is essential in diabetic foot management, including metabolic control, infection treatment, restoration of vascular circulation, and effective wound care strategies. In recent years, negative pressure wound therapy (VAC/NPWT) has become widely used and has shown superior outcomes compared to standard wound care. VAC therapy promotes wound healing by removing exudate, reducing edema, controlling bacterial load, and stimulating granulation tissue through controlled negative pressure. Proper wound bed preparation, including debridement and infection control, is crucial before initiating VAC therapy. Foam selection and pressure settings should be individualized according to the patient’s condition. VAC therapy is particularly effective in large ulcers, amputation sites, and infected wounds. Numerous clinical studies have demonstrated that VAC accelerates wound closure, enhances granulation tissue formation, and reduces the need for major amputations. However, its disadvantages include relatively high cost and potential tissue damage if used incorrectly. In conclusion, VAC therapy has become an important component in the management of diabetic foot ulcers when combined with appropriate patient selection and a multidisciplinary treatment approach.
