Synopsis

Chronic diseases negatively affect individual quality of life and place an increasingly significant burden on healthcare systems. The development of care models that are person-centered, multidisciplinary, and preventive in nature has become essential. Nurses play a vital role in this transformation process. In older adults, geriatric assessment supports the delivery of individualized nursing care and the preservation of functional abilities. Home healthcare and palliative care services offer holistic support models addressing cultural, ethical, and psychosocial dimensions particularly for individuals with limited functional capacity or those in the terminal stage. The integration of digital health technologies into nursing practice reduces hospital admissions and enhances treatment adherence.
