Synopsis

Diabetes is recognized as a major public health problem worldwide. It is a lifelong chronic disease that can affect multiple organs and is the third most common chronic condition after cerebrovascular diseases and malignant neoplasms. This non-communicable disease poses a significant burden on human health. It is estimated that approximately two-thirds of individuals with diabetes are also affected by hypertension. Elevated arterial blood pressure significantly contributes to the increased incidence of both microvascular and macrovascular complications in diabetic patients. Moreover, the coexistence of these two major risk factors results in an approximately fourfold higher risk of cardiovascular disease compared with normotensive and non-diabetic individuals. In recent years, traditional nursing approaches based on monitoring patients’ blood glucose and blood pressure have been widely used in the care of individuals with diabetes. However, since most diabetic patients also present with hypertension and are prone to cardiovascular and cerebrovascular diseases, the effectiveness of this approach remains limited. Therefore, the use of scientifically based nursing models and care practices in the active treatment process of patients is of great importance. Such models enable nurses to provide evidence-based guidance on patients’ lifestyle and medication habits, thereby contributing to the improvement of their quality of life.
